Job Summary
We are seeking a dedicated and compassionate Mobile Nurse Practitioner (Community Travel in Hamilton) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including pediatrics, home care, and hospital environments. This role requires a strong understanding of medical terminology and proficiency in electronic health record systems such as Cerner and Epic. The Nurse Practitioner will be responsible for assessing patient needs, developing treatment plans, and managing ongoing patient care, with a focus on pain management and dementia care.

We are seeking a dedicated and adaptable Travelling/Mobile Nurse Practitioner (NP) to join our dynamic healthcare team for Community Work in Hamilton. This role involves travel to various clinical sites to provide primary and specialized care to diverse patient populations. The ideal candidate will have a strong clinical background, the ability to work independently, and a passion for providing high-quality, compassionate care in both urban and rural settings.

Key Responsibilities:

  • Conduct comprehensive health assessments, including physical exams, patient histories, and diagnostic evaluations
  • Diagnose and treat acute and chronic conditions
  • Prescribe medications and therapeutic treatments in accordance with scope of practice and applicable regulations
  • Collaborate with physicians, specialists, and multidisciplinary teams to coordinate patient care
  • Provide patient education and counseling on health maintenance and disease prevention
  • Maintain accurate, thorough, and timely documentation in electronic medical records
  • Travel regularly to assigned sites, including community clinics, remote facilities, and partner locations
  • Ensure compliance with clinical protocols, privacy standards, and regulatory requirements
  • Participate in clinical meetings, training, and continuous improvement initiatives

Qualifications:

  • Master’s degree in Nursing (Nurse Practitioner stream) from an accredited program
  • Current Nurse Practitioner license in good standing
  • Authorization to prescribe medications
  • Minimum 2 years of experience in a nurse practitioner role preferred
  • Valid driver’s license with vehicle and willingness to travel frequently
  • Exceptional communication, critical thinking, and organizational skills
  • Experience in primary care, urgent care, or community health is an asset

Compensation :

  • Starting salary: $111,286 or $53.50/hr (may increase to 130k/year after 3 months depending on experience)
