Support Team (RPN) – Dementia Care

Front Line Work Force Inc.

Location: Markham, ON
Work Environment: Memory care / dementia support program

Pay: $30.00 per hour
Job Type: Part-time

Schedule

All shifts available – Sunday to Saturday

  • Day shifts
    • Afternoon shifts
    • Evening shifts
    • Overnight shifts
    • 8-hour shifts

Flexible availability required (including weekends and holidays).

About the Role

Front Line Work Force Inc. is currently seeking a dedicated and compassionate Support Team Lead (RPN) to support individuals living with Alzheimer’s and related dementias.

In this leadership role, you will oversee the medical care and well-being of members, while also working alongside the support team to provide a hands-on, universal approach to care.

This position is ideal for someone who is passionate about dementia care, enjoys leading a team, and thrives in a supportive, engaging environment.

Key Responsibilities

Clinical & Leadership Duties

  • Administer medications and oversee the medication program
    • Delegate tasks appropriately to team members
    • Conduct medication checks and liaise with pharmacy as needed
    • Assist with member assessments and care planning
    • Communicate medical updates and concerns with families
    • Lead and support the team on all medically related matters
    • Schedule and document caregiver meetings from intake to discharge

Resident Care & Support

  • Provide companionship and emotional support
    • Assist members with toileting in a respectful and dignified manner
    • Support dining (meal prep, serving, feeding assistance, clean-up)
    • Assist with grooming, bathing, and personal care as required
    • Ensure members are safe, comfortable, engaged, and well cared for
    • Monitor hydration, nutrition, and overall well-being

Program & Daily Living Support

  • Participate in and lead group programs and activities
    • Create a warm, welcoming, and engaging environment
    • Maintain cleanliness through light housekeeping and laundry
    • Ensure daily assigned tasks are completed for each member
    • Conduct regular safety checks and member counts

Team Collaboration

  • Maintain open communication with team members
    • Use internal systems/tablets for documentation and communication
    • Address concerns and escalate appropriately
    • Follow all health & safety policies and procedures

Qualifications

  • Registered Practical Nurse (RPN) or Registered Nurse (RN)
    • Member in good standing with the College of Nurses of Ontario
    • Minimum 1 year of relevant nursing experience
    • Experience working with Alzheimer’s and dementia populations
    • Strong leadership, assessment, and decision-making skills
    • Excellent communication and interpersonal skills

Requirements

  • Current CPR and First Aid Certification
    Vulnerable Sector Police Check
    Mantoux (TB) Test clearance
    • Ability to lift up to 35 lbs
    • Physically able to stand, sit, and bend for extended periods
    • Willingness to receive annual flu vaccination (unless medically exempt)

  • Second language an asset
    • Experience leading group programs and activities
    • Strong organizational and time management skills
    • Ability to manage multiple priorities in a fast-paced environment
